This is a contract between The Pets Hotel Port Melbourne Pty Ltd (hereinafter called The Pets Hotel) and the owner whose signature appears below (hereinafter called the owner).

The owner agrees that day care dogs must be over 3 months of age and that all dogs over 7 months must be spayed or neutered (desexed).

• The owner agrees that day care dogs must check-in and check-out on leash.
• The owner agrees that day care dogs must be in good health. A current C5 vaccination certificate must be sighted.
• Bookings are essential.
• Medication can be administered at a rate of $1.50
• The owners accept that Dogs will only be checked-out to the registered applicant. If an alternative arrangement is required, the registered applicant must provide any variation to this in writing.
• The Pets Hotel accepts cash, visa or MasterCard.
• If a dog is found not suitable for day care, membership will be terminated and any monies in credit will be refunded in full.
• All dogs are cared for in accordance with the Government of Victoria code of practice. The Pets Hotel, representatives and employees will be indemnified by the dog owner against any actions or claims arising from any loss, injury or canine influenza that a dog sustains or
• causes to another dog or person while in the care of The Pets Hotel.
• If veterinary attention is required for a dog the owner is liable for all expenses incurred.  Should a dog damage The Pets Hotel property the owner will be billed for the replacement cost.

By signing this contract the owner certifies to the accuracy of all information given about the said pet on the pet details form.
